Project designers can use form display logic to make forms or surveys only available under specified conditions. Similar to how branching logic is used to make fields available under certain conditions, form display logic makes entire instruments available under certain conditions.

Accessing Form Display Logic
Form Display Logic can be found on the Designer page.
Begin by selecting "Edit Form Display Logic".
Form Display Logic Settings
A pop-up window will appear. We will discuss most of the steps here, but we recommend that you read the instructions in this window as there may be information that we do not include here.
In the pop-up window, there will be a few checkboxes.
Keep forms enabled if they contain data - This may be particularly useful if you are trying to hide forms after you have already started data collection. Without selecting this box, the forms that do not meet your specified criteria will become inaccessible even if data has already been collected on them.
Hide forms that are disabled - Without this enabled, the forms will still be visible but greyed out and inaccessible. With this option enabled, the form will be entirely hidden.
Enable support for Survey Auto-Continue - If you are using Form Display Logic for surveys that are impacted by survey auto-continue settings, and you would like the instrument to be skipped over unless the record meets your specified criteria, then select this setting to enable survey auto-continue support.
Form Display Logic Conditions
By default, if the form is not selected under any conditions, it will be available at all times.
Example:
Let's say Form 1 is a screener where we are determining eligibility, and we do not want my data entry staff to have access to any other forms unless that participant has screened eligible. I have selected Forms 2-8 to only be available as long as a participant has "Yes" selected for the [eligible] field.
Now, since this participant was marked as ineligible, those forms are greyed out and inaccessible.
Then, if they are marked as eligible, I can access Forms 2-8 again.
